Instructions

• Please fill in all the sections. Entries in any other format or with unfilled sections will not be evaluated. Be brief and highlight only salient points.
• Additional information, if any, should be provided in Annexure(s).
• One organization can submit multiple nominations and can apply under different categories. Use a separate nomination form for each entry.
• Shortlisting will be based on evaluation of the information submitted by the organization.
• All entries will be evaluated by a jury based on relevance of the initiative, novelty of the idea, sustainability, scalability, and overall contribution to patient safety.
• The decision of the jury will be final.
• Once a nomination is submitted, the organization will receive a 25% discount promo code on one regular registration.
• Shortlisted organizations must submit the details of the contact person attending the conference and award function. The attendee must be registered through the online portal.
