Patient Safety Declaration 3.0
- The governance and leadership team of every hospital shall create and foster a Culture of Safety within the organization.
- Every hospital shall appoint a designated Patient Safety Officer and establish a Patient Safety Committee with members from clinical and non-clinical teams, meeting monthly.
- Evidence-based care practices must be implemented and monitored regularly through structured clinical audits.
- All employees shall be trained on patient safety, including patients’ rights and responsibilities.
- Patients shall be educated and empowered on safety-related matters.
- A system for reporting adverse events must be implemented, ensuring active surveillance and data availability.
- International Patient Safety Goals (IPSG) / WHO Patient Safety Solutions (PSS) must be adopted and participation in the National Patient Safety Program is encouraged.
- Hospitals shall collaborate and share information on adverse events, solutions, and lessons learned to enhance risk prevention.
- Hospitals shall collect and monitor data for key patient safety indicators (not limited to the listed items).
